WAIVER

Waiver of Liability and Release Agreement ~ Smoke Detector

The undersigned acknowledges receipt of ______ smoke detector(s) from the Baltimore County Professional Fire Fighters Association, IAFF Local 1311. This smoke detector is being provided free of charge as a public service to Baltimore County residents, and is not intended to be given to businesses of any type (including rental units and home-based business such as day-care centers, foster homes, etc.). The undersigned acknowledges that he/she is not relying on any expertise of the Fire Fighters Association in supplying the device, and further recognizes that the Fire Fighters Association makes no warranties or representations regarding the fitness of the device for its intended purpose, or the appropriateness or effectiveness of the device's installation. The undersigned expressly releases and waives any and all claims which it now has or may have in the future against the Baltimore County Professional Fire Fighters Association arising from the receipt or installation of said smoke detector(s).

(*Station must return this form to the Union Office for replacement smoke detectors.)
