Forms
For the Union's Survivor's Fund. This link NOW takes you to a
Word doc with TWO half-page forms. Send one to the Union and keep the
second copy in your personal files. These new forms request more
information, so if you haven't filed one with the office yet, don't bother to
call and check on who you currently have listed -- just
complete, sign, and send in the new form today.
For the Fire Department form which is handled
by the Retirees'
Association,
Note that their form must be notarized, which I can do for you.)
You'll also want to contact both the Insurance Office (410-887-2568)
and American Income Life (800-433-3405).

GRIEVANCE Form
in MS Word ...
This is set up in Microsoft Word, and is formatted
to print off of my LaserJet 4 Plus. Consequently, when you open it,
the document may need reformatting. Margins are top/bottom .25 and
left/right .5. It's been cleaned up a bit (10/07); basically what you
want to do is type over the X's and make sure the whole thing looks neat
when you're finished. Good luck!
